For and in consideration for my participation in SOCCER TEMPO
I, the undersigned, hereby assume full responsibility for myself, my heirs, executors, administrators, insurers for any risk of injury, death or property damage due to the result of my participation in this event, or any event hosted by SOCCER TEMPO LLC, and unconditionally and voluntarily release and waive any and all rights and claims that I may now, or in the future have against SOCCER TEMPO LLC, and its respective subsidiaries, agents, employees, directors, officers, representatives, contractors, sponsors, affiliates, successors and assigns, (collectively, the "Released Parties").
I understand that signing this release is a full and final release applying to all unknown and unanticipated injuries, deaths, damage or loss, and injuries arising out of my participation in the league games, tournament games, practices, travel, or social activities, and the undersigned waives all rights or benefits which the undersigned now has or in the future may have under the terms of section 1542 of the Civil Code of the State of California. I acknowledge that I am aware of and assume the risks in participating in an athletic event of this type. I hereby fully release and discharge the Released Parties from any and all claims for injuries, death, damage or loss which I may have or which may accrue to me from my participation in this event or any events hosted by SOCCER TEMPO LLC, even though that liability may arise out of the negligence, carelessness or other conduct on the part of the Released Parties. I will assume my own medical and emergency expenses in the event of an accident of injury resulting from my attending this event or any event. I further agree to indemnify and hold harmless and defend the Released Parties from any and all claims, damages, liabilities and/or costs (including attorney’s fees) arising out of, connected with or in any way associated with my participation in the event or any event hosted by SOCCER TEMPO LLC.
I understand that the Released parties are not responsible for any inclement weather, which may affect the event or any event, and release and waive any and all claims against the Released Parties for damages, liabilities or loss that may arise therefore.
I understand that violation by me of the registration rules will cause my team to forfeit all games played while I am a participant. I understand that any behavior on my part contrary to the company rules and player code of conduct may result in my dismissal from the event.
I have read the above and understand that it and my signature below confirms my full acceptance to be bound by the terms stated. I certify that all information provided on the registration forms is true and correct. I certify that I have read, understand, and agree to abide by the company rules and player code of conduct.
I understand that the league, it’s officers and or any team shall not be responsible for ensuring that health coverage is in force or the validity of any personal identification or personal health coverage. I have read and agree to maintain my own medical insurance coverage protecting me against injury while participating in the Soccer Tempo leagues, tournaments, its games, practices, training, camps, clinics, tours, social activities, and travel to and from these activities.
I hereby give my consent for emergency medical care prescribed by a duly licensed Doctor of Medicine. This care may be given under whatever conditions are necessary to preserve the life, limb, or well being of myself and I hold all persons rendering such care harmless for all such efforts on my behalf.
This document is intended to be as broad and inclusive as permitted by law, and if any portion hereof is held invalid, it is agreed that the balance shall not withstanding, continue in full legal force and effect. This document embodies the entire agreement with respect to these matters and supersedes any previous or contemporaneous negotiations or agreements. Each of the undersigned agrees that no promise or inducement has been offered except as herein set forth, and that this document has not been executed based upon statements or representations by any person or entity other that those set forth herein. I have been advised to contact an attorney prior to execution hereof. This document shall be governed by and construed under the laws of the state of California, without regards to conflicts of law principles therein.
I HAVE READ AND FULLY UNDERSTAND AND VOLUNTARILY EXECUTE THIS WAIVER, RELEASE OF ALL CLAIMS AND INDEMNIFICATION DOCUMENT.
